There is a major clinical focus to the role. Responsibilities of the role will comprise of assessment, planning delivery and evaluation of care given to patients and their families as the caseload and role requires. The CNSp will be required to work in close collaboration with nursing and medical colleagues and the wider MDT. The CNSp in Hepatology will participate and disseminate research and audit and provide consultancy in education and clinical practice to patients, their families, nursing colleagues and MDT colleagues and others as required. Enabling patient empowerment to achieve optimum well-being using appropriate care planning/case management in collaboration with medical and MDT. colleagues will provide the foundation for the role. This will also involve advocating on behalf of the patient and promoting self-care through appropriate strategies. Patient education will include specific topics families and or carers will be integral to this health promotional role
