Description:
Inflammatory Bowel Disease (IBD) is a chronic relapsing, remitting condition and requires ongoing management. Beaumont Hospital has a cohort of 2000 IBD patients and also manages a transition service for adolescents with IBD between Crumlin & Beaumont. The post holder is expected to demonstrate team leadership and management in the maintenance and development of a high-quality service to patients with IBD. They will be responsible for the day to day management of patients with IBD in the inpatient and clinic settings.

- To be an effective Practitioner able to establish and maintain high quality evidence based nursing service
- Accept and manage referrals and provide an inpatient IBD service pathway
- Co-ordinate new treatments and refer to infusion facilities or homecare services
- Actively participate in planning and carrying out care for patients attending the IBD service
- Provide support to the family/ significant other, though listening, problem solving and education.
- Provide outreach support for the patient at home
- Attend IBD Nurse Led Outpatient Clinics and co-ordinate visits for each patient.
- Assess patients regarding their need for referral to specialist services.
- Report such patients promptly to the consultant
- Co-ordinate relevant services in preparation for treatment
- Attend multidisciplinary team meeting
- Educate patients re: diagnoses and treatments
- Foster good working relationships with all members of the clinical team, which are both professional and supportive
- Maintain accurate clinical records complying with legislation and best practice.
- Demonstrate specialist knowledge of and clinical expertise in the delivery of patient care.
- Be aware of all hospital policies and procedures and collaborate with other health care professionals to ensure that these are observed
- Be actively involved in the preparation and implementation of policies and nursing guidelines in relation to the management of care to IBD patients.
- Demonstrate leadership ability and good communication skills.
- Attend and participate in transition clinics.
- Be responsible for the provision of ongoing nursing assessment and care of patients post discharge i.e. during outpatient and day clinics, hospital admissions and home visits
- Attend Biological Virtual Clinics and work with IBD Consultants in current and future switch programmes in biosimilars.
- Manage the IBD Nurse advice Line
- Provide an efficient and effective high quality service, respecting the needs of each patient.
- Act as a resource/ contact person for IBD patients attending other departments within the hospital or outside the hospital for treatment.
- Provide patients and family/ significant others with contact details for the IBD nurse specialist service
- Be aware of existing resources/ services, which help patients and their family/ significant others, eg social services, support groups, entitlements.
- Acts as an advocate for patients undergoing clinical trials, providing information as appropriate and communicating with research staff.
- Participate in team discussions regarding treatment forIBD patients.
- Facilitate development of information leaflets for patients and families.
- Create and maintain an environment conducive to evidence based teaching, learning and practice.
- Act as an effective role model, mentor and leader of staff.
- Identify and utilise teaching, learning and practice opportunities in the clinical setting and encourage problem based learning strategies based on patient health problems within services providing care to patients with IBD.
- Ensure the availability of appropriate information/educational resources for patients and families aiming to encourage independence, the prevention of illness and the promotion of health for all patients with IBD
- Have a creative outlook enabling progress to be made towards the ultimate objective of reaching standards of excellence in the delivery of nursing care and in the co-ordination of multi-disciplinary care.
- Maintain and update knowledge of relevant professional development and all current trends in the speciality of IBD nursing, and an awareness of future trends.
- Attend and participate in staff development programmes and represent IBD Nursing as a speciality at meetings and conferences, as required.
- Discuss present performance and future needs with the Directorate Nurse Manager.
- Ensure that his/her own practice is compliant and up to date with “best practice”, which is reflected in research and development.
- Participates in the regular audit of his/her own clinical practice
Ensure the development and maintenance of a quality assurance programme which is patient centered utilising up to date research findings
- Use such research findings to influence care within the specialty.
- Set objectives and develop policies, procedures and standards for the IBD nursing services based on current nursing research and relevant research from other disciplines.
- Identify areas within the specialty relevant to nursing practice, which would benefit from research.
- Promote an awareness of research-based practice amongst staff.
- Participate in departmental research related to the specialty.
- Will participate in all current audits within Beaumont Hospital relevant to the IBD service.
- Assist in the production of an annual department report
- Evaluate audit results and research findings to identify areas for quality improvement in collaboration with nursing management and MDT colleagues.
- Provide leadership in clinical practice and act as a resource and role model for IBD nursing practice
- Establish collegial partnerships and in the context of IBD, contribute to the professional development of students, peers, colleagues and others through consultation, education, leadership, mentorship and coaching
- Work with, support, advise and help build up knowledge of expertise of the primary care team in providing an appropriate system of care to meet the needs of the patient and their family/carers
- Develop and maintain good collaborative working relationships and communication with all members of the speciality’s MDT and other health care professionals within and outside the service.
- Participate in the implementation of the national clinical care programmes and their agreed models of care for patients and their families as they relate to the role
- Use specialist knowledge to support and enhance general nursing practice
- Actively participate in change management in order to develop the service based on patient needs.
- Develop a flexible and innovative approach, forming links with other specialists both nationally and internationally.
- Attend Gastroenterology conferences and committees, and actively participate in lecturing and dissemination of knowledge
