To co-ordinate and support an efficient, effective and high-quality service that provides continuity of care and quality of life to patients diagnosed with Lung cancer.
Plan, implement, deliver, and evaluate care for a caseload of lung cancer patients in collaboration with Respiratory Consultants, Respiratory CNS and Lung CNS. This role will cross both MMUH and CHB.
Act as an informed resource for health and social care professionals involved in the care of patients with Lung Cancer.
Facilitate and contribute to education and training for health and social care professionals in collaboration with relevant stakeholders.
Support the collection of data in relation to Lung Cancer patients.
Attend and participate in the weekly Lung Cancer MDT at MMUH.
Improved quality of life for patients and their families
Improved co-ordination of diagnostic services to Lung Cancer patients.
To facilitate education and training for health and social care professionals in collaboration with relevant stakeholders.
To act as an informed resource and link person for patient and family carers and for health and social care professionals involved in the managing Lung Cancer patients
To link with Cardio thoracic surgery service MMUH, PHNs, Palliative Care.
