Description:

My client are seeking a compassionate and dedicated Social Worker to join their team, providing support to individuals with intellectual disabilities and mental health challenges. The role involves assessment, care planning, advocacy, and coordination of services to promote the well-being and social inclusion of clients. The Social Worker will work collaboratively with a multidisciplinary team to ensure holistic care tailored to individual needs. This role is based in Wicklow and is commutable from South Dublin

 

Key Responsibilities:

 

  • Assessment and Care Planning:
  • Conduct thorough psychosocial assessments to understand the needs, strengths, and challenges of individuals with intellectual disabilities and mental health conditions.
  • Develop personalized care plans in collaboration with the client, family, and relevant stakeholders.
  • Regularly review and update care plans to reflect changes in the client’s circumstances or needs.
  • Advocacy and Support:
  • Advocate for clients’ rights, promoting access to appropriate services, benefits, and resources.
  • Provide emotional and practical support to individuals and their families to help them navigate social, health, and legal systems.
  • Empower clients to make informed decisions about their care and support services.
  • Crisis Intervention and Risk Management:
  • Respond to crisis situations, implementing de-escalation techniques and ensuring immediate safety.
  • Identify and manage risks, working closely with mental health professionals and support services to prevent harm or deterioration.
  • Coordination of Services:
  • Collaborate with a multidisciplinary team including psychiatrists, psychologists, occupational therapists, and support workers to deliver coordinated care.
  • Refer clients to relevant external services (housing, employment, education, health) and liaise with community organizations for additional support.
  • Maintain strong relationships with families, carers, and external agencies to ensure seamless care.
  • Monitoring and Reporting:
  • Maintain accurate, up-to-date case records and documentation, ensuring compliance with regulatory and legal requirements.
  • Monitor progress, evaluate outcomes, and report on the effectiveness of interventions.
  • Participate in case reviews, meetings, and supervision sessions to ensure quality and consistency in service delivery.

 

Essential Qualifications and Experience:

  • A Bachelor’s or Master’s degree in Social Work
  • Must be CORU registered
  • Previous experience (working with individuals with intellectual disabilities and/or mental health issues would be beneficial
  • Experience in case management, crisis intervention, and multi-agency collaboration.
  • Understanding of intellectual disabilities, mental health disorders, and their social, emotional, and behavioral impacts.
  • Familiarity with local community resources, support services, and benefits
