Transplant Coordinator

Description:

Be a key member of the Heart and Lung Transplant team, facilitating the flow of scheduled and unscheduled patients for transplantation

Lead, manage, triage, support pre-admit, admission / transfer of all Transplant patients in consultation with the key stakeholders of the multidisciplinary team.

Assist in streamlining the pathway for all Transplant assessment (scheduled and unscheduled) admissions to / within the hospital, ensuring the most efficient use of bed capacity, inpatient throughput and timely patient discharge.

Provide clinical leadership to develop and support all staff involved in managing pre, intra operative transplant patients.

Receive organ donor offers and effectively coordinate potential heart and lung transplant/s

Receive neonatal valve donor offers and effectively co-ordinate retrieval

Co-ordinate onward referral, seamless transfer / bi-directional flow to and from referral centres.

Assist with on-going development of pathways for patients awaiting assessment and listed for Transplantation.

Drive the extension of clinical pathways for all patients to and from MMUH / all referral sites in accordance with local and national guidelines.
